This plan had a HRC meeting on 3/29/23 with no 6 month follow up.If a physical restraint is used on an unanticipated, emergency basis, §§ 6400.194 and 6400.195 (relating to human rights team; and behavior support component of the individual plan) do not apply until after the restraint is used for the same individual twice in a 6-month period.For Individual #1 and all individuals with approved emergency restrictive procedures in their plan, the Behavior Support Plan (BSP) will be updated every 6 months to align with HRC meetings for re-approval of restrictive procedures (if indicated based on criteria data). In addition, a separate spreadsheet has been created to record the dates of when HRC approved the restrictive procedures and the next date that the plan/restrictive are up for review/re-approval. All individuals who were out of compliance will be reviewed during the March 2024 HRC meeting on 3/27/24. 6400.62(c)There was a soap container on the kitchen sink with no ingredients listed on the bottle. There were six spray bottles in the locked closet that had various poisons in them. Each bottle listed the contents, window cleaner, surface cleaner and so on but no ingredients were listed on the bottles.Poisonous materials shall be stored in their original, labeled containers. 6400.46(b)Staff #2's fire safety training was not completed on an annual basis. The training was conducted on 7/19/21 and then again on 8/31/ 22, which was one month past the annual due date.Program specialists and direct service workers shall be trained annually by a fire safety expert in the training areas specified in subsection (a).An excel spreadsheet was created on March 6, 2023 by the Chief Executive Officer. All WCS staff were listed on the spreadsheet, along with the dates of their last mandatory trainings and dates for when their next training should take place. This spreadsheet will be reviewed monthly at the WCS core team meeting and information will be shared with the Program Supervisor.
