The Risperdone blister pack was filled on 8/17/23. The 31-pill blister pack had one pill on number 8. No date or initials to determine a proper pill count from 8/17 to 9/20. Staff reports there had been a second pack. Staff should initial and date pack/punch pill in linear order to ensure proper count.
Divalproex is to be given at bedtime and was filled on 8/24/23. 3 pills were punched out on number 24, 30 and 31 and not in linear order so the count cannot be determined.
Clonazepam to be given in the morning was filled on 8/17/23. 9 out of 31 pills are remaining with numbers 15-31 punched out. Because of this there is no way to determine the actual pill count as it doesn't match the MAR. | A medication record shall be kept, including the following for each individual for whom a prescription medication is administered: Name and initials of the person administering the medication. | 1. It is the responsibility of the Program Supervisor and Program Director to ensure that medications are being properly documented as they are administered. |
