Upon discharge from the hospital, Individual #1 was not prescribed Lorazepam. When reviewing Medication Administration Records (MAR) and medication blister packs for March, Staff #1 discovered a blister pack that contained Lorazepam 1mg tabs (Generic for Ativan). Take 1 tablet by mouth three times a day at 8AM, 4PM and 8PM located in the medication closet. Staff #1, who was reviewing the MAR and medications for March advised Staff #2 that this medication had been located in the medication cabinet and asked Staff #2 if it should be added to the Medication Administration Record. Staff #2 advised Staff #1 if the medication was not on the Medication Administration Record, it should be added. Staff #1 added the medication the MAR as it was believed that Individual #1 was prescribed this medication. Staff #1 and Staff #2 did not verify if this medication was prescribed before adding it to the MAR. This medication was administered to Individual #1 on March 1, 2023, at 8AM, March 2, 2023, at 4PM and 8PM, March 3, 2023, at 8AM and 8PM, March 3, 5, 6, 7, 8, 9 and 10, 2023 at 8AM, 4PM and 8PM. The administration of this medication was discovered on March 8, 2023, by the individual's DDDT team and reported to Individual #1's Psychiatrist. Individual #1's Psychiatrist began titration of the medication on March 11, 2023. Individual #1 should not have received this medication as it was not prescribed to the individual. The blister pack of medication that was located in the home had been delivered on December 29, 2022, while Individual #1 was hospitalized. The blister pack was placed the medication cabinet and was not returned to the pharmacy. Individual #1 was abused in that a medication that was not prescribed to Individual #1 was administered to the individual. Staff failed to contact the prescribing physician to determine if the individual should receive the medication. Staff did not contact the physician prior to administering the medication to ensure that other medications that Individual #1 is prescribed would have a negative interaction with the Lorazepam.Abuse of an individual is prohibited. Individual #1 was prescribed Clonazepam 0.5mg, take 1 tablet by mouth three times a day begin on 3/11/23. This medication is not administered as prescribed. This medication was not documented as administered on 3/12/23 8at 8AM. The medication was documented as administered on 3/17/23 at 4PM, 8PM, 3/1/23 at 8AM and 4PM and 3/19/23 at 8AM. The inspector was present at the home on 3/17/23 at 12PM; it is unknown why all medications documented as administered after this time were documented. There was only one blister pack that is utilized to administer this medication. Staff reported that all does are administered from one blister pack and when it is empty, the next pack started. The times, 8AM, 4PM and 8PM were had written next to the label on the blister pack. The blister pack is dated 3/10/23 and contained 5 unused pills. If pack originally contained 22 pills. There were signatures documenting that 19 pills had been administered 1 dose not documented as administered and 4 refusals with 5 pills remaining in the blister pack. It is unknown if Individual received this medication as prescribed or why future dates were documented as administered. If administered properly from the start date to the date of inspection, 24 pills would have been required and there should not have been any remaining in the blister pack. It is unknown if the medication is being administered as prescribed or why the medication is documented as administered for future times and dates. Individual #1 was prescribed Lithium Carbonate 300mg, take 1 capsule by mouth three times a day begin on 3/10/23. The blister pack in the home was dated 3/1/23 and had 8AM, 4PM and 8PM had written next to the label. The medication was documented as administered on 3/17/23 at 4PM, 8PM, 3/18/23 at 8AM, 4PM and 9PM and 3/19/23 at 8AM. The inspector was present at the home on 3/17/23 at 12PM; it is unknown why all medications documented as administered after this time were documented. There was only one blister pack that is utilized to administer this medication. Staff reported that all does are administered from one blister pack and when it is empty, the next pack started. The blister pack in the home contained 31 tablets on 3/1/23. If the blister pack is being utilized for three doses of medication daily, on 3/17/23 at 12PM, there should have been no pills in this blister pack. The blister pack contained 17 pills. It is unknown if the medication is being administered as prescribed or why the medication is documented as administered for future times and dates. Individual #1 was prescribed Haloperidol 5mg tabs, take 1 tablet twice a day for 7 days then take 1 tablet every morning for 7 days. This medication is documented as administered on 3/18/23 at 8AM. The inspection too place on 3/17/23 at 12PM. There was one blister pack of the medication dated 3/8/23 available in the home. Next to the label on the blister pack, 8AM and 8PM was handwritten on the blister pack. There was only one blister pack that is utilized to administer this medication. Staff reported that all does are administered from one blister pack and when it is empty, the next pack started. The blister pack was started on 3/8/23 at 8PM. Seven 7 pills remained in the blister pack. There were two refusals of this medication documented on the Medication Administration record. There were 21 pills in the original blister pack on 3/8/23. If administered as prescribed, there should be 6 pills remaining in the blister pack. Based on the number of pills remaining in the blister pack, this medication is not being administered as prescribed. Individual #1 was prescribed Benztropine Mesylate 0.5mg, take 1 tablet by mouth twice a day for seven days then take 1 tablet by mouth every morning for 7 days beginning on 3/8/23. Individual #1 was prescribed Benztropine 0.5mg tab, take 1 tablet by mouth twice a day. This medication was discontinued on 3/8/23. Individual #1 refused this medication on 3/4/23. The refusal is documented on the Medication Administration Record under the date and time that the medication should have been administered, however there is no documentation that the refusal shall was reported to the prescriber. Individual #1 was prescribed Haloperidol 5mg tabs, take 1 tablet twice a day for 7 days then take 1 tablet every morning for 7 days. The refusal shall be reported to the prescriber as directed by the prescriber or if there is harm to the individual.All staff had their med privileges revoked and had to redo the new medication administration course that came out in January. 6400.186Individual #1's Individual Service Plan (ISP) including revisions is not being implemented. Individual #1 requires 2:1 staffing at all times per the individuals ISP as of 1/26/23, there is no additional information regarding supervision and where staff need to be located in proximity to Individual #1. Individual #1 during behavioral episodes, will throw staff out of the home and staff will remain out of the home until the behavioral episodes end. On at least one occasion, staff reported remaining out of the home from 1:00AM until 7:00AM. Staff remain outside of the door of the home, on at least on occasion, one staff will move to a lower level of the stairs outside of the home when Individual #1 targets that staff leaving Individual #1 with inappropriate staff supervision.The home shall implement the individual plan, including revisions.this plan of staff staying outside of the door was created by the DDTT team and reviewed with all staff by the DDTT behavior specialist. The staff were informed that if the individual kicks them out of the house that they are to stay in the hallway right outside her door and to check on her every 10 minutes. there were two chairs that were bought for the staff to utilize when she kicks them out. 07/05/2023 Implemented
