| 6400.64(a) | There was a line of a black substance consistent with mold on the caulk around the tub. The line was approximately 10 inches long. Staff did try to correct the issue during inspection but was unable to clear the substance. | Clean and sanitary conditions shall be maintained in the home. | Shared Support South maintenance went to individual's home to remove line of black substance and to caulk the tub again.
