The review of medications prescribed to treat symptoms of a psychiatric illness completed 1/31/20, 4/24/20, and 7/20/20 did not include the reason for prescribing the medication. | If a medication is prescribed to treat symptoms of a psychiatric illness, there shall be a review by a licensed physician at least every 3 months that includes to document the reason for prescribing the medication, the need to continue the medication and the necessary dosage. | The Medication Review/Psychiatric documentation sheet has been revised to reflect direct questions to be completed by the physician completing the medication review. All staff will be trained and will sign off on the revisions. The revised documentation sheet will be emailed to the Licensing representative for review. [Upon completion, the CEO or Designee will ensure that all staff have been trained on the revised Medication Review/Psychiatric documentation sheet. Immediately and at least quarterly for 1 year, program specialists will audit the current psychiatric medication review documentation\ for all individuals to ensure, timely, completion with required information.
