6400.141(c)(14) | On the annual physical exam dated 2/21/22 for Individual #1, the section citing information pertinent to diagnosis and treatment in case of emergency is blank. | The physical examination shall include: Medical information pertinent to diagnosis and treatment in case of an emergency. | Provider Coordinator has re-submitted ( 5/10/22) to the PCP office the annual physical for individual #1 , so that it can be updated to reflect what was reviewed during the initial physical - which includes information pertinent to diagnosis and treatment in case of emergency which was initially left blank. The PCP will be in the office on 5/11/2022 and has expressed that the update will be completed and faxed back by 5/16/22. The program coordinator was retrained on what is required to be completed during a annual physical. |

6400.165(g) | Individual #2 had a psych medication review on 2/22/22, however there were no previous psych med reviews on file. | If a medication is prescribed to treat symptoms of a psychiatric illness, there shall be a review by a licensed physician at least every 3 months that includes to document the reason for prescribing the medication, the need to continue the medication and the necessary dosage. | The program coordinator, has reached out to the psyche office several times and faxed over the psyche medication review sheet to the psyche office and the provider agency is waiting to get the paperwork back. As a backup we have also reached out to the PCP to complete the review of the psyche medication as well. Staff, (5/9/22) were retrained on what is required for a psyche medication review appointment. |
