6400.166(b)Individual #1's MAR is not being initialed when medications are administered; August 1st and 2nd meds were absent from the blister packs, but were not indicated as given on the MAR - Acetaminophen 500mg (PRN) was absent from the blister pack on 8/1, but not initialed on the MAR as administered - Propranolol (PRN) was absent from the blister pack, shown to have been given 3x, but it was not initialed on the MAR as administeredThe information in subsection (a)(12) and (13) shall be recorded in the medication record at the time the medication is administered.Acetaminophen 500 mg and Propanol are both PRN medications and are not dispensed monthly by the pharmacy. The Acetaminophen 500 mg was dispensed on 6/13/2022 and the Propranolol was dispensed on 3/21/2022 . Therefore, the corresponding numbers next to medications do not represent the date but the number of pills dispensed at the time. As per safe medication administration practices and OLALUS LLC policies the medication administrator is to administer medications based on the next available medication in the blister pack and not the corresponding date. Acetaminophen was administered on 7/13/2022 and documented as such. Upon investigation it was found that the medication administrator that administered the Acetaminophen on 7/13/2022 dropped a pill. It is OLALUS policy that if a medication is dropped it is now considered "dirty" and must be discarded. The next pill available in the blister pack should be administered, the Agency Nurse should be made aware, and the incident should be documented in the progress note (please see attached supporting documentation). Propranolol was administered on 4/7/2022, 4/14/22, 4/28/22 and documented as such (please see supporting documentation). 08/02/2022 Implemented
