6400.64(a)There were dark substances consistent with dirt in the stove in the kitchen.Clean and sanitary conditions shall be maintained in the home. Upon discovery, the oven was cleaned thoroughly (Pictures will be submitted on a separate email) CSS also inspected other residential facilities to make sure all appliances including oven are clean and sanitary. staff were retrained in the proper cleaning procedure and maintaining a clean environment for our Individuals. A cleaning check list has also been developed and is being utilized by staff on a daily basis. the site supervisor does weekly monitoring to ensure that adequate clean and sanitary conditions are met in each home. 6400.68(b)The water temperature in the home was 132.2 in the kitchen and 140 in the bathroom on the second floor. Hot water temperatures in bathtubs and showers may not exceed 120°F. On January 15, 2019 Commonwealth Supportive Services contacted J. Williams General contractor who are specialized in heating and cooling systems. However in an effort to ensure the safety of our Individuals, CSS strictly followed our hot water protocol which requires staff to mix the water and hand check it before allowing Individuals to take their shower. Staff have continually remained in the bathroom with clients when taking their shower to provide adequate safety supervision. On 1/28/19 our contractor installed a brand new mixing valve and regulated the water temperature below 120 degrees Fahrenheits. Since then the water temperature in home has been with the requirement of the 6400 regulations. CSS provided training on managing hot water. A temperature log was developed and staff were trained to check the water temperature daily using a well calibrated thermometer and log accordingly. The site manger reviews the temperature log every day. Maintenance staff will check the water heater weekly to ensure it working correctly. The Direct of Quality Assurance and the CEO shall provide ongoing monitoring and oversight of this plan. 05/14/2019 Implemented
6400.76(a)There was a thick layer of lint the lint trap. Furniture and equipment shall be nonhazardous, clean and sturdy. Upon discovering the lint in the dryer, the house supervisor immediately removed the lint from the dryer. The site supervisor inspected the dryers in all other sites to make sure they were free from lint. Direct Support Professionals were trained on the importance of keeping the lint trap clean after every load and the hazard associated with lint in the dryer. A note informing staff to clean lint from the dryer after every load was also posted on the dryer cover. Going forward, the DSP shall document at the end of every shift that the dryer was check and cleaned. the site supervisor shall monitor the documentation to ensure staff are doing the shift checks. the Director of Quality Assurance shall monitor this plan to ensure compliance with the regulation. 05/14/2019 Implemented
