| 6400.43(b)(4) | On July 1st 2020, Sunset Support Serives, Inc. submitted a "Self-Inspection and Declaration Tool" to the Department to increase capacity by adding a home to an existing 55 Pa. Code Chapter 6400 license which included sworn documents that the information contained on the submitted document was true and correct, that the agency is responsible for compliance with all applicable statutes and regulations, and that knowingly providing inaccurate information may lead to enforcement action up to and including revocation of the agency's license to operate.
55. Pa Code Chapter 6400.81(J) requires a bedroom shall have doors at all entrances for privacy.

Pa Code Chapter 6400.81(J) that the home had a door installed for privacy. | The chief executive officer shall be responsible for the administration and general management of the home, including the following: Compliance with this chapter. | Licensing has temporarily revoked Sunset's abilities to self-inspection our own properties before opening a new site location and will require a licensing representative from ODP come out to complete an inspection. Prior to contacting licensing to complete an inspection, Sunset CEO and Owner will jointly complete our own inspection prior to licensing arrival to ensure that the properties are in compliance with ODP regulations. |

Path was covered in snow and ice. | Outside walkways shall be free from ice, snow, obstructions and other hazards. | Sunset's Agency trainer will send out an agency wide training on the importance of maintaining all exits in case staff/individual need to emergency egress. This training will have a signoff sheet which staff will be required to sign off. |
