6400.165(g) | The review of medications prescribed to treat symptoms of a psychiatric illness completed 5/6/19 for Individual #1 does not include reason for prescribing the medications. | If a medication is prescribed to treat symptoms of a psychiatric illness, there shall be a review by a licensed physician at least every 3 months that includes to document the reason for prescribing the medication, the need to continue the medication and the necessary dosage. | Medication reviews will be completed by a licensed physician at least every 3 months as prescribed by 6400.165(g). The individual had medication reviews completed on 4/09/19, 5/06/19, and 7/17/19. The April and July medication reviews were completed accurately to reflect all necessary information.
Program Specialists will review all medication reviews by 11/15/2019 to ensure full compliance with this regulation. In the future, Program Specialists will be responsible to review this document upon completion by the physician. Any errors or omissions on the form will be addressed in a timely manner to ensure compliance.
Further, the Compliance Coordinator will conduct audits of all records at least every 6 months. Part of the audit will include a review of the physician¿s medication review for compliance. All employees responsible for these checks and reviews will be trained by 11/15/2019. |
