Risperidone 2 mg tablet, take one tablet by mouth every morning and evening, Guanfacine HCL ER 3 mg, take one tablet by mouth every morning and Biotin 5,000 MCG capsule, take one capsule by mouth daily prescribed for Individual #1 were not initialed as administered on 7/14/17 at 8:00AM. | The information specified in subsection (a) shall be logged immediately after each individual's dose of medication. | The Direct Support Professionals (DSPs) for the home will be trained on the regulation and agency expectations at their next monthly meeting by 11/17/2017. The Program Manager for the home will also be trained on the regulation and agency expectations at the next monthly meeting on 11/03/2017. The expectation of the DSPs is to immediately initial the MAR (log) after each individual's dose of medication is administered. The expectation of the Program Manager is to review the MAR regularly throughout the month to ensure the MAR is documented properly. At the end of the month, the Program Manager will sign that they reviewed the MAR at the bottom of the document prior to filing it in the permanent record. |
