All supervisors will be trained on the regulation and agency expectations, as well as the modified form at their next supervisory meeting on 11/03/2017. The modified form will be used to document the medication review on that date and for all future medication review appointments to ensure the reason for prescribing the medication, the need to continue the medication and the necessary dosage are documented.
