6400.163(c) | The psychiatric medication reviews for Individual #1 completed 10/31/16, 1/24/17, 4/20/17, 7/17/17 and 9/27/17 did not include the entire necessary dosage, the frequency of administration was not included. | If a medication is prescribed to treat symptoms of a diagnosed psychiatric illness, there shall be a review with documentation by a licensed physician at least every 3 months that includes the reason for prescribing the medication, the need to continue the medication and the necessary dosage. | The PHYSICIAN MEDICAL REVIEW form has been modified to provide additional space and direction for its completion. All supervisors will be trained on the regulation and agency expectations, as well as the modified form at their next supervisory meeting on 11/03/2017. Individual #1 has an appointment scheduled for 12/18/2017. The modified form will be used to document the medication review on that date and for all future medication review appointments to ensure the reason for prescribing the medication, the need to continue the medication and the necessary dosage are documented. [Upon completion, a designated staff person trained in the requirements of psychiatric medication reviews as per 6400.163c, shall audit all individual's psychiatric medication review documentation to ensure all required information is included and all individuals are administered medications as prescribed.
