6400.161(a) | The staff in Individual #1 home does not assist him as needed with the self-administration of his medication. According to staff statement at home, they stated that they only drive him to the pharmacy to pick up his medication. As medication that has expired as early as 4/23/20, it seems that staff are not reviewing the medications at any regular interval. As the most recent medication list in the home was from 2019, it seems that staff are not assisting in making sure that individual # 1 knows which medications he is to take. | The home shall provide an individual who has a prescribed medication with assistance, as needed, for the individual's self-administration of the medication. | Manager purchased a month medication dispenser for Individual 1. Staff or manager assist Individual 1 with the filling and dispensing of medications the first of the month. Individual 1 continues to order his 90 day refills, picks up and pays for his medications independently. CVS provides a print out of all his current and discontinued medications for staff and Frank to reference upon filling monthly dispenser. |

6400.163(h) | Individual #1 , a self-medication individual had the following expired medication in the home that was also still being used: glimepiride 1 mg taken for diabetes expired 6/7/21, quinapril hci 40 mg tab with prescription instructions to take 1x daily for hypertension expired on 6/7/21 and there were two bottles of terazosin hci 2mg with prescription instructions to take 1x daily which expired on 4/23/20 and 7/8/21. Medication amlodipine besylate 10 mg expired 6/7/21, however this was not being used as there was a non-expired medication of the same name present in the home. | Prescription medications that are discontinued or expired shall be destroyed in a safe manner according to Federal and State statutes and regulations. | Discontinued and expired medications were removed from the property on day of inspection. Excess and discontinued medications are disposed of at police station and CVS pharmacy with disposal drop boxes. Frank will get a current med listing when he picks up his 90 day refill at CVS. Staff will compare the list with medications on site. Frank will staff oversight fills a month pill dispenser. Any changes in medications due to doctor visits will be communicated to the Residential management team by email or text to ensure timely and proper removal from the premises. |
