We prepopulate the reason for the prescribed medication due to physicians unwillingness to complete forms. If there is any other reason the med is prescribed outside of what we have populated on the form the physician will typically add that information into the form and we update the form accordingly to reflect the doctor's notes from the previous appointment. By doing this method it has allowed us to get the information we need from the physician without any delay. |
