We prepopulate the reason for the prescribed medication due to physicians unwillingness to complete forms. If there is any other reason the med is prescribed outside of what we have populated on the form the physician will typically add that information into the form and we update the form accordingly to reflect the doctor's notes from the previous appointment. By doing this method it has allowed us to get the information we need from the physician without any delay. 03/24/2022 Implemented

The plan of correction will be that water temp will be checked in the morning and the evening to make sure that water temp doesn't exceed 120 F. We will also be reaching out to the management company to have the water temp down to 120 F in the building. The person that will be measuring the water temp on an daily basis will be the Team Lead that is over the house as well as the DCS on shift in the evening and morning shifts and monitored by the PS as well. 08/23/2019 Implemented
