6400.66On 10/4/23 at 1:18PM, the light at the rear exit from garage was not operable. There is not another source of light in this area.Rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps and fire escapes shall be lighted to assure safety and to avoid accidents. 6400.101On 10/4/23 at 1:19PM the door leading from the kitchen to the basement had turn locking mechanism on the kitchen side of the door preventing egress from the basement when engaged. There is not another exit from the basement.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. Immediate Plan of action: The door handle was switched around to ensure the turning locking mechanism was facing inside the basement. 6400.74The interior stairs, leading to the second floor of the home, did not have a non-skid surface.Interior stairs and outside steps shall have a nonskid surface. Nonskid strips are necessary in the home to avoid slips and falls which could potentially be harmful. Sr Program Specialist Loren Weadon scheduled for the maintenance department to put the non-skid strips on the stairs. The maintenance department completed the task on 7/22/18. Our regulatory checklist which are completed by staff on each shift prompts the staff to check all stairways have a non skid surface. If carpet or non skid mats are in need of repair staff are to complete a maintenance request and the problem should be fixed within 24 hours of the request. Maintenance request are submitted electronically and are sent directly to the maintenance department. [Within 30 days of receipt of the plan of correction, the CEO or designee shall train all staff persons working in community homes of the aforementioned process.
