6400.68(b)At 11:42 AM, the hot water temperature in the bathtub in the bathroom on the third floor of the home measured 125.7 degrees Fahrenheit. Hot water temperatures in bathtubs and showers may not exceed 120°F. The water temperature at this site is regulated by an individual on site water tank. On 3/18/15 the water temperature was lowered to 115 F by WRC Maintenance staff. The water temperature at this site will be monitored by WRC Maintenance staff on a monthly basis to ensure it does not exceed 120 F. 03/27/2015 Implemented
6400.82(f)The bathroom on the third floor of the home did not have a trash receptacle.Each bathroom and toilet area that is used shall have a sink, wall mirror, soap, toilet paper, individual clean paper or cloth towels and trash receptacle.The trash receptacle typically kept in the bathroom was placed around the corner in the bedroom while the bathroom floor was being cleaned and was mistakenly not returned to the bathroom. This violation was corrected on site at the time of inspection. Staff will receive additional training from Program Specialist on the requirements outlined by regulation 6400.82(f) during the March 2015 Monthly Staff Meeting. 6400.110(e)The three story home serving three individuals does not have interconnected and audible smoke detectors on the three floors of the home or an automatic fire alarm system that is audible throughout the home.If the home serves four or more individuals or if the home has three or more stories including the basement and attic, there shall be at least one smoke detector on each floor interconnected and audible throughout the home or an automatic fire alarm system that is audible throughout the home. The requirement for homes with three or more stories does not apply to homes licensed in accordance with this chapter prior to November 8, 1991. Current smoke detectors at the site were previously installed by ADT and were operationally interconnected at the time of our last monthly Fire Drill held on 2/17/15, but at the time of testing during inspection the alarm only rang at the detector being tested. According to ADT, this may be due to the fact that the wires in the fire alarm panel had sustained damage. A First Alert 2 in 1 Smoke and Carbon Monoxide Alarm was purchased and installed by WRC Maintenance staff on Friday March 6th, 2015. This alarm is wirelessly interconnected to communicate with alarms located on other floors of the residence. This alarm also includes an exclusive talking feature that announces the type and location of danger, as well as a photoelectric sensor that stops most false alarms. Maintenance has instructed staff on how to use the one button test alarm function. Photographs of the alarm itself, as well as the receipt of purchase have been submitted for your review. 03/27/2015 Implemented

6400.141(c)(4)Individual #1's pre-admission physical dated 9-23-11 did not include a hearing screening . The individual's admission date was 4-6-12.(4) Vision and hearing screening for individuals 18 years of age or older, as recommended by the physician. WRC provided the inspector with a letter from the doctor that performed the physical on 9/23/11 stating that Individual #1's hearing was within normal limits at the time of the physical. The physical used at the time of intake stated that Individual #1's ears were within normal limits but this should have been changed to hearing. In the future WRC only admit an individual who has a WRC physical form completed and signed by a doctor at time of admission. Our physical states "hearing screening" not ears. We will set up a system where the program director will monitor all new admission physicals prior to admission. After program director reviews the physical it will be turned over to the compliance director for a second and final review. This will ensure compliance with regualation 6400.141(c)(4). WRC had developed a new Pre-Admission checklist which clearly states all documents needed, based on 6400 Regulations prior admitting an individaul into WRC. Both the residentail director and program specialists were trained on this document and its contents by Nicole Varga, compliance director on 1/8/13. 01/22/2013 Implemented
