6500.31(a) | The "Rights" signed by Individual #1 on 1/19/13 did not identify the full Individual Rights per Chapter 6500 Regulations. The Rights not identified include but are not limited to the following: 33(a) "An individual may not be neglected, abused, mistreated or subjected to corporal punishment," 33(e) "An individual has the right to privacy in bedrooms, bathrooms and during personal care," 33(h) "An individual has the right to reasonable access to a telephone and the opportunity to receive and make private calls, with assistance when necessary," and 33(l) "An individual has the right to be free from excessive medication." | Each individual, or the individual's parent, guardian or advocate if appropriate, shall be informed of the individual's rights upon admission and annually thereafter | The old form was corrected and reviewed with each 6500 individual verifying that they were informed of their rights. this was done be by November 14, 2013. Prevention from occurring again: a central forms database that will be maintained by the administrative assistant or designee. This database will ensure all the most current forms reflect the most current regulatory requirements and are the only ones used by agency personnel. It will be the responsibility of the program specialist to inform individuals of their rights and to obtain their signatures indicating they have been informed of these rights. |
