6400.151(c)(2)Direct Service Worker #1, date of hire 06/05/20, had a tuberculin skin test by Mantoux method read by a certified medical assistant on 06/04/20. The physical examination shall include: Tuberculin skin testing by Mantoux method with negative results every 2 years; or, if tuberculin skin test is positive, an initial chest x-ray with results noted. Tuberculin skin testing may be completed and certified in writing by a registered nurse or a licensed practical nurse instead of a licensed physician, licensed physician's assistant or certified nurse practitioner. Point of Caring, Inc. was in violation of 55 PA Code Chapter 6400.141(c)(6) by failing to ensure the tuberculin skin test was completed and certified by a licensed practical nurse or registered nurse. This violation occurred due to lack of training for the house administrators and direct care staff, and failure to implement a 2-point checking system. In order to avoid future violations of 55 PA Code Chapter 6400.151(c)(2), Point of Caring will provide training to all staff, which will include the regulation as well as the proper credentials of the medical personnel, who are completing and certifying the tuberculin skin test. This training will be provided upon the hiring of all new staff, as well as all current staff. The training, for all current staff, is to be completed by 4/15/21 or earlier if any staff have a physical due prior to this date. In addition, Point of Caring will attach a letter to all employee physical forms, as a reminder for all staff to request the appropriate medical personnel, and to assist in their personal review of their physical, upon completion. Upon completion of the physical exam, the staff return the form to the human resource staff responsible for the staff physical forms. The human resource staff will review the physical form, to serve as a 2-point checking system. An individual has the right to receive, purchase, have and use personal property. [Individual #1] has access to his belongings, however due to [Individual #1's] behavior of property destruction and clothing destruction a staff member must be present when he utilizes his dresser, in order to prevent [Individual #1] form ripping all of his clothing on a daily basis as noted in his documentation and Restrictive Procedure. [Individual #1's]Restrictive Procedure Plan details the procedure to allow [Individual #1] the right to have access to his clothing, while preventing complete loss of all of his clothes. Staff assist [Individual #1] in safely choosing clothing when he wants and desires. [Individual #1] cannot affor to purchase new clothing every day and must have clothing to be in the common areas of his home and in the community. Point of Caring, Inc. will continue to work with [Individual #1] in developing coping skills/strategies in order for him to not destroy all of his clothing and shoes.[On 5/9/17, Individual #1's restrictive procedures/SEE Plan and Fade plan were updated to include Individual #1's dresser is kept in the individual's locked bedroom closet and the procedures to ensure Individual #1 has access to belongings at any time with staff supervision following the fade plan.
