Their ISP states they need to be supervised while at home. Individual #2 does not need to be within the line of sight but should be within hearing distance and checked on periodically. Individual #2's assessment and ISP for their ability to use or avoid poisonous material do not match. Individual #2's assessment dated 1/22/2021, their functional skill assessment indicates that they are a 4 which is independently or 100% of the time, and the individual does not require any assistance to complete the task as it relates for them to use or avoid poisons. The assessment also states that Individual #2 does not try to ingest or get into anything that's not food or drink related. He knows the difference. Their ISP states under general health and safety risks Individual #2 is supervised due to his sensory needs: he loves to smell things, taste different liquids and feel different objects. This could put him at risk. Under safety precautions it states that Individual #2 does not have any understanding of safety but does not generally try to get into any non-food items. The individual plan shall be revised when an individual's needs change based upon a current assessment.The individual plan shall be initially developed, revised annually and revised when an individual's needs change based upon a current assessment.Individual #1: The assessment has been updated to match the ISP that the individual has unsupervised time while her provider steps outside up to 5 min. The individual can also be unsupervised in her room or other areas of the home while the provider in in the home. This was done on 05/03/2021.
