Each individual's record must include the following information:
(1) Personal information, including:
(vi) A current, dated photograph.
Individual #1's picture on file was dated October 2018. Pictures are to be current; taken annually or if there is significant change in the individuals appearance. | Each individual's record must include the following information: Personal information, including: (i) The name, sex, admission date, birthdate and Social Security number. | Individuals pictures will be updated annually or more often if the individual changes significantly in appearance. All pictures were updated immediately. |
