At the time of the inspection the trash receptible was full and the lid was not able to be closed, as the garbage bags filled the entire bin. In addition, there were 2 full bags of garbage on the ground next to the trash receptacle.Trash outside the home shall be kept in closed receptacles that prevent the penetration of insects and rodents.The sanitation workers skipped a week of pick up due to the inclement weather at the time of the agency's inspection. All trash was taken out the same day and disposed at the agency's dumpster by the house supervisor to prevent insects and rodents. 6400.80(a)Outside walkways shall be free from ice, snow, obstructions, and other hazards. At the time of inspection the back porch steps were covered with snow. Outside walkways shall be free from ice, snow, obstructions and other hazards. The outside walkway was covered with snow. The snow remover personnel did not get to the residence in a timely manner to clear out the snow at the time of inspection. The house supervisor on shift and the program manager were able to clear the outside walkways on the same day. 01/16/2024 Implemented
6400.141(c)(3)Immunizations for individuals 18 years of age or older as recommended by the United States Public Health Service, Centers for Disease Control. The TDAP shall be given to an individual every 10 years. Individual #1 did not have any documentation that a TDAP vaccine was administered.The physical examination shall include: Immunizations for individuals 18 years of age or older as recommended by the United States Public Health Service, Centers for Disease Control, Atlanta, Georgia 30333. The Tdap was not included in the immunization record of the individual. The doctor has no documentation that TDap has administered. The program specialist scheduled a TDap shot with his PCP immediately. The shot was given on the 26th of January 2023. 01/26/2024 Implemented
6400.141(c)(6)The individual physical shall include a TB test upon admission and every two years thereafter. The individual #1 date of admission was 7/5/23 and he did not have a TB test documented at that time. The only TB test in his record is from 8.8.23 which was after his admission.The physical examination shall include: Tuberculin skin testing by Mantoux method with negative results every 2 years for individuals 1 year of age or older; or, if tuberculin skin test is positive, an initial chest x-ray with results noted. A TB test was not completed or in the immunization record of the individual before admission into UCCH program. During our monthly audit, it was reviewed that the individual has no TB result on file.
