6400.68(b)The hot water temperature in the home was tested and found to be 134.6f Hot water temperatures in bathtubs and showers may not exceed 120°F. Community Support Facilitator completed several tests of the water temperature in this individual¿s home, specific to the shower/bath water, over the course of 6 weeks following the onsite inspection where the bathroom sink water temperature was tested and read at 134.6F. During those subsequent checks, the hot water temperature in the shower/bath never tested over 120 degrees Fahrenheit. The hot water coming from other water sources in the home did test over the regulated amount, however the regulation is specific to hot water in the bath tub/shower and the only individual living in this home has an assessment where it is documented he can regulate water temperatures independently. Values Into Action¿s Service Director will ensure that the water temperature in the bath/shower is continuously tested on a monthly basis to prevent any instances of non-compliance in all homes. Staff responsible for completing water temperature testing will be trained to specifically test the water temperature in the bath/shower area, to ensure compliance specific to this regulation by 2/1/2021. 6400.216(a)Individual's program books were in an unlocked cabinet in the dining room area of the home. An individual's records shall be kept locked when unattended. Service Director immediately (11/18/2020) ensured that these items were relocated to a locked filing cabinet within the individual¿s home. A new cabinet was purchased, with a lock system, to ensure the individual¿s records are kept safe and confidential, while also ensuring the staff and individual himself have easy access to the documentation as needed. This new system was implemented on 1/18/2021. Service Director and Community Support Facilitator completed an assessment of all other homes to ensure program books and other sensitive information is stored in a locked system on 12/22/2020 and will continue to monitor ongoing compliance via monthly inspections of homes completed by Community Support Facilitator, and reviewed by Service Director. Values Into Action has contacted this individual's property manager to request this issue be corrected. The maintenance testing of water temperature displayed temperatures below the regulation; however Values Into Action has requested to still move forward with installation of a part that would allow this individual's specific unit to have the water temperature adjusted further to ensure ongoing compliance with this regulation, and more importantly ongoing safety for the individual. Values Into Action immediately addressed this issue with Property Management. Due to Property Management not being able to further regulate the water temperature of the Individual's unit, Values Into Action has scheduled a consult with a professional plumber, requesting the faucet valve be adjusted to not allow as much hot water to flow into the applicable area. This is scheduled for 4/20/17. Moving forward, Values Into Action will ensure that water temperatures in these areas do not exceed the 120 degrees by testing water temperature on a quarterly basis at minimum. Moving forward, Values Into Action will ensure that all furniture and equipment is in regulatory compliance by having the Team Coordinators complete quarterly home inspections at minimum. Values Into Action has recommended to Individual #2 that he have his Primary Care Physician perform the prostate exam for now, to ensure his overall health. Values Into Action will continue to support, educate, and encourage Individual #2 to complete this exam ASAP.
