6400.67(b) | On 11/1/23 at approximately 11:15AM, there was a tear that measured approximately one foot long by three inches wide in the carpeting at the entrance to Individual #1's bedroom posing a tripping hazard. | Floors, walls, ceilings and other surfaces shall be free of hazards. | Tape is being used to secure the carpet to the floor temporarily to alleviate tripping hazards. As this home is located in an apartment building that is not owned by Achieva, we have to work in conjunction with the owners of the property to make any needed repairs. We are estimating that complete repairs will be made within a month. Achieva's Facilities VP has contacted Morgan Properties regarding the tear in the carpet. A work order request was electronically placed and a request confirmation was received on 11/7/23. (Verification to follow via email to ODP) Achieva's Facilities department will ensure continued contact with Morgan Properties that the repair is made as soon as possible. Associate Director of CSL will correspond with the Facilities VP weekly until repair is made. |
