In ChartMeds, the diagnosis for each medication has been entered and will appear on the electronic MAR for each medication for each administration. We will begin piloting ChartMeds in February and institute ChartMeds across our department in April 2022. Until that time, we do have documentation that provides the diagnosis/reason each medication is taken for that is kept in the medication binder with the paper MARs. This has always been our practice, unfortunately we did not present this during the inspection process. 01/12/2022 Implemented

[Repeat Violation 10/14/2020 - 10/16/2020]The program specialist shall provide the assessment to the individual plan team members at least 30 calendar days prior to an individual plan meeting.The Training & Development Manager has been designated as a gatekeeper to provide oversight and ensure that Assessment information is shared with the Team at least 30 days prior to the ISP meeting date. This Manager will work with the Specialists to create and maintain a chart of annual Assessment & ISP due dates for each person supported. The Manager will alert the Specialist when Assessments are due and will then collect the completed Assessment from the Specialist. The Manager will send the updated Assessment to the Team at least 30 days prior to the ISP meeting date. The Manager will keep documentation that this has occurred within the appropriate timeframe and provide this documentation for the person¿s record. 01/12/2022 Implemented

6400.112(a)There were no fire drills completed in February, 2015; March, 2015 and April, 2015. An unannounced fire drill shall be held at least once a month. On the first of the month each Community Homes Supervisor will submit the previous month's completed fire drill to the Community Homes Specialist. The Community Homes Specialist will immediately submit the completed form to the office for filing.
