6400.165(g) | The review of medications prescribed to treat symptoms of psychiatric illness completed 5/13/19 for Individual #1 did not include the need to continue the medication. | If a medication is prescribed to treat symptoms of a psychiatric illness, there shall be a review by a licensed physician at least every 3 months that includes to document the reason for prescribing the medication, the need to continue the medication and the necessary dosage. | The Psychiatric appointment form from 5-13-19 for individual #1 will be updated by the Psychiatrist 3-12-2020 to document the need to continue the medication. The Program Specialist will review all Psychiatric appointment summaries for individuals with medications prescribed to treat symptoms of psychiatric illness to assure the need to continue as documented. At least quarterly for 1 year the site supervisor will review 25% sample of completed psychiatric appointment forms to ensure aforementioned process is working and that required.
