6400.68(b) | On 4-20-17 at 2:14 PM, the hot water temperature at the bathtub in the bathroom along the main hallway measured 123.4 degrees Fahrenheit. | Hot water temperatures in bathtubs and showers may not exceed 120°F. | The hot water tank was adjusted to a lower temperature and the temperature was measured again on 5/4/17 and the reading was 110 degrees. A copy of the hot water temperature log indicating the date and the hot water temperature will be submitted to ODP.New digital thermometers have been ordered for all PMHS facilities as evidenced by the description of the item and evidence of the order being placed. The evidence will be sent to ODP. All sites will continue to test and document the hot water temperature every month on the log. All Program Specialists will be retrained on the requirement to ensure that the hot water temperatures are maintained below 120 degrees by 5-26-17. Evidence of the training will be submitted to ODP.[Immediately and at least weekly until hot water temperatures do not exceed 120°F and then continuing at least monthly, hot water temperature shall be measured at all bathtubs and showers in all community homes. Documentation of measurements shall be kept and reviewed at least quarterly by designated management staff person. Within 30 days of receipt of the plan of correction all staff person responsible for measuring hot water temperature shall be trained on the policies and procedures to ensure hot water temperatures in bathtubs and showers do not exceed 120°F at all times. 6400.213(1)(i) | The photograph in Individual #1's record was not dated. | Each individual's record must include the following information: Personal information including: (i) The name, sex, admission date, birthdate and social security number. (iii) The language or means of communication spoken or understood by the individual and the primary language used in the individual's natural home, if other than English. (ii) The race, height, weight, color of hair, color of eyes and identifying marks.(iv) The religious affiliation. (v) The next of kin.
