There was a light in Individual #1's closet in the bedroom that was not operable. 6400.67(a)On 4/13/2022, at 10:25AM there was a door knob sized hole in the wall behind the door in Individual #1's bedroom. There was a door knob sized hole in the wall behind the door in the bathroom on the first floor of the home. In addition, there was a towel rack that was missing the towel hanging rod and one of the two wall brackets in the bathroom on the first floor of the home.Floors, walls, ceilings and other surfaces shall be in good repair. The holes have been patched and the towel rack was repaired. Moving forward if there are holes in the walls staff will immediately put a request in online to maintenance. 04/28/2022 Implemented

A first aid kit shall contain antiseptic, an assortment of adhesive bandages, sterile gauze pads, a thermometer, tweezers, tape, scissors and syrup of Ipecac, if an individual 4 years of age or younger, or an individual likely to ingest poisons, is served. The program specialist with be sure that the essential items that are easily taken scissors, thermometers, tweezers duplicates of these will be had on site and will be added to the first aid kit as needed. But on a monthly basis the first aid kit will be monitored the staff will document that all the required items are present in the kit. And will replace any needed items. All staff will be trained on the required items needed in a first aid kit. Proof of this will be by having Program Specialist Trained by 4-16-19 as evidenced by the training roster. Moving forward a spreadsheet was implemented putting a check mark denoting when the client signs, staff signs and Program Specialist signs the ISP review. This was forwarded to the Lead State Inspector who conducted the monitoring. [Individual #1 signed and dated January 2015 ISP review on 4/4/16. Immediately, the program specialist will review all individual 3 month reviews for past 6 months to ensure the program specialists and individual have signed and dated the reviews upon review of the ISP. All reviews that have not been signed or dated will be reviewed and accurately dated. At least quarterly for one year, the director will review a 25% sample of ISP reviews to ensure the program specialist and individual are signing and accurately dating the ISP review upon review of the ISP. Within 30 days of receipt of the plan of correction the Director will train program specialists on completion of aforementioned spread sheet and the policy and procedure on ensuring that the program specialist and individuals sign and date the ISP reviews upon review.
