6400.185(b) The ISP for Individual #1 clearly states that he requires 2:1 staffing at all times. On 05/01/2015 a licensing representive conducted an unannounced inspection at this home. The Licensing Representative arrived at approximately 1 pm and was informed by staff #1 that Individual #1 eloped at approximately 8:30 am on that day and therefore he was not at home. Staff #2 was following him in the agency van throughout the community while staff #1 stayed at the house to call 911 non-emergencies. Staff #2 returned with Individual #1 at approximately 3pm. Licensing Representative was also informed that staff #2 took Individual #1 to the Mall to look for CD's as he enjoys music. A trip to the Mall was standard if Individual #1 agreed to get in the van. Hence, Individual #1 did not have 2:1 staffing when he was in the community and at the Mall. The ISP shall be implemented as written.Individual #1 has very challenging behaviors related to his neuropsychological development. As a result a behavioral plan was developed by a behavioral specialist that includes a protocol to respond when individual #1 elopes. That protocol, which we have taught to staff, requires us to have one staff person stay at the home to call non emergency 911 and wait to see if individual #1 returns to the house. The second staff person pursues individual #1 at a distance that will not make individual #1 run even further, and tries to convince individual #1 to return home. Unfortunately, this protocol, which should have been attached to the ISP by the supports coordinator, was never attached. That has been corrected. At the same time, we do not condone staff taking it upon themselves to work outside of the guidelines of individual #1¿s approved plans. This is what happened when staff stopped to buy a CD for individual #1 on the way back to the house. This is not the correct procedure and will not be tolerated. A procedural review will be completed with Staff #2, which is the first step in our disciplinary process. All Staff working in this home will continue to be trained on his supervision needs. All staff working in individual #1¿s home will review the behavioral plan and ISP of individual #1 again with management before an upcoming June 4, 2015 team meeting for individual #1. On June 4, 2015 individual #1¿s team met to plan additional supports for Individual #1 that meet his complex needs. Additional Behavioral Specialist hours have been added to the individual¿s plan. We have begun weekly team conference call to assure all team members are on the same page and to monitor the effectiveness of planned supports. 6400.67(b)On July 10, 2014, the window in Individual #1¿s bedroom was broken and glass was falling out of the window. Floors, walls, ceilings and other surfaces shall be free of hazards.Individual residing in the home had thrown an object at the window the morning of July 10th. Maintenance was notified, but was in another county at the time. The window was repaired that afternoon. Although staff had removed glass that was loose and had fallen out of the window frame they failed to cover the window. Residential supervisor discussed with staff the need to cover the window to protect from any possible injury while they waited for the repair 07/10/2014 Implemented
6400.81(i)On July 10, 2014 the window blinds in Individual #1¿s bedroom window were broken.Bedroom windows shall have drapes, curtains, shades, blinds or shutters.
