6400.67(b) | There was a protruding screw tip in a cubby hole where the front door handle backs up against the wall when the door it opened into the living room. | Floors, walls, ceilings and other surfaces shall be free of hazards. | On 2/7/24, a protruding screw tip was found on the front door handle. At the time of the inspection, the screw was immediately removed by the Chief Administrative Officer (Attachment 3). On 2/12/24, the manager of the home completed a home review showing that all areas and surfaces inside the home were in good repair and that there were no hazards (Attachment 4). A training was completed on this regulation by the Chief of Supports and Services on 2/15/24 (Attachment 5). |

6400.68(c) | Water testing done on 7/11/23, then not again until 10/20/23 (9 days late). | A home that is not connected to a public water system shall have a coliform water test by a Department of Environmental Resources¿ certified laboratory stating that the water is safe for drinking purposes at least every 3 months. Written certification of the water test shall be kept. | On 2/6/24, it was discovered that the well water testing for the home had been completed 9 days late. On 10/19/23, Water sent a letter stating that this was due to a scheduling conflict, and as corrective action, testing has been scheduled for every 2 months in 2024 (Attachment 6). The dates of the appointments have been pre-scheduled for the remainder of the year. The most recent well water test was completed on 12/22/23, which was within compliance (Attachment 7). A training was completed on this regulation by the Chief of Supports and Services on 2/15/24 (Attachment 5). |
