In order to address the violation listed above, PDSI implemented the following:
PDSI hired a Director of Family Living, Samantha Gatto, who started 10/17/2016. (Attachment #4). She will be responsible (as a second set of eyes in conjunction with the Family Living Specialist) for tracking due dates for programming and medical documentation for all family living individuals.
Kristy Meyer, Vice President of Quality Management, revised the Primary Book Review Form to correspond with Family Living Regulations (Attachment #8). This document will be utilized by the Family Living Specialist and/or the Director of Family Living at least quarterly.
The Program Specialists and FLP Director were trained on the Primary Book Review Form by Kristy Meyer, Vice President of Quality Management on 11/28/2016 (Attachment #9).
Validation: Primary Book Review Forms were completed for all FLP individuals (Attachment #10). |
