6400.76(a) | There is a green, rocking, reclining chair located in the 4 season's room of the house, that was not sturdy. The frame of the chair wobbled from side to side when lightly pushed. It wobbled approximately 6 inches from one spot to the next.
Individual #1 has a restrictive behavior plan in place in which furniture is to be secured to the floor to prevent to prevent Individual #1 from moving the furniture. A large, brown, cushioned chair in the four seasons room was not affixed to the floor to prevent it from being moved with little effort. The front, right leg of the chair was not affixed to the floor and the rear right leg was loosely attached to the floor.
A large, cushioned, red chair in the four seasons room was able to be lifted off the ground on the left side without much effort. The front left leg was not affixed to the floor and the nail in the rear left leg, ripped from the floor when moved. | Furniture and equipment shall be nonhazardous, clean and sturdy. | 1. On 8/7/2019 the chair was removed from the home. (Attachments 7 and 8)
2. Management staff were re-trained on regulation 6400.76(a) that states that furniture and equipment shall be nonhazardous, clean and sturdy on 8/28/19 and 8/29/19 by Erica Kershner and Anna Edling of the PDS Quality Management Department. (Attachments 2 and 3)
3. The community home review was revised to include terminology of furniture being non wobbly. (Attachment 4)
4. Management staff were trained on the revised community home review form on 8/28/19 and 8/29/19 by Erica Kershner and Anna Edling of the PDS Quality Management Department. (Attachments 3 and 5)
5. Validation- community home reviews were completed for each home, to ensure that all homes are in compliance with this regulation. A sample of these completed forms are attached as validation. (Attachment 6) |
