6400.15(a) | The provider's license expired 2/8/2016. The home's self inspection was completed 11/10/2015. | The agency shall complete a self-assessment of each home the agency operates serving eight or fewer individuals, within 3 to 6 months prior to the expiration date of the agency¿s certificate of compliance, to measure and record compliance with this chapter.

PDSI implemented the following:
1. Kristy Meyer, Vice President of Quality Management, and all Quality Management Coordinators were retrained on the 6400 regulations, and in particular re-review regulation 6400.15(a) regarding self-assessments (Attachment #11).
2. For 2017, the Central Region 6400 self-assessments will be completed between August 8, 2016-November 8, 2016 by Kristy Meyer, Vice President of Quality Management, and the Quality Management Department. They are currently scheduled for the month of September 2016 (Attachment #12). |
