6400.144 | Three Medications for Individual #1 were in the medication box, but were not on the Medical Administration Record (MAR)
-Acetaminophen tab 325 mg (take 2 tabs by mouth every 6 hours as needed for headache or pain)
-Acetaminophen tab 325 mg (take 1 tablet by mouth 4 times daily as needed for pain)
-Loratadine tab 120 mg (take 1 tablet by mouth every 12 hours as needed for nasal congestion) | Health services, such as medical, nursing, pharmaceutical, dental, dietary and psychological services that are planned or prescribed for the individual shall be arranged for or provided.
| This regulation is important because if the medication was needed as a PRN to treat the symptoms it was prescribed for, it would not have been available to do so.
What occurred was that the PRN medication for cold symptoms was used in October but after that, the current Rx ran out, therefore did not populate on the pharmacy MAR and was apparently not renewed even though there was an active physicians order.
The medication was taken from the home and a new order will be secured from the physician, filled, and returned to the medication inventory in the home.
Going forward, each community home supervisor or designee will do weekly checks of medication inventory comparing the medication to the MARs.
If a medication does not appear on the MAR, the med will be removed from the home until the discrepancy can be reconciled.
If a medication is on the MAR and not present in the home, it will be ordered and picked up from the pharmacy right away. If there is no current order for that medication than it will be discontinued on the MAR. |

It was discovered through research of previous months MARS that these medications were discontinued on 10/25/19 for acetaminophen and on 11/4/19 for loratadine. | Prescription medications that are discontinued or expired shall be destroyed in a safe manner according to Federal and State statutes and regulations. | This regulation is important to help prevent misuse of medication and potential medication errors.
