6400.64(a)The adaptive bathroom had mold on the grout between the tiles. Clean and sanitary conditions shall be maintained in the home. Director contacted JEVS contracted building maintenance agency, JDB. JDB assessed the situation and made the recommendation to replace floor and tiles on the wall. The mold was getting under the tiles due to missing grout. This should help prevent mold from developing in the shower along with regular cleanings. The direct care staff will do a formal cleaning at least every other week to ensure a clean and sanitary shower. The associate director, supervisor of the site, will do monthly inspections of the site to ensure the house is clean and any repairs that need to be addressed are reported and taken care of. 08/17/2017 Implemented
