6400.110(e)At the time of the inspection, the smoke detectors in the home were not interconnected. The smoke detectors on the second floor were not interconnected with the rest of the home.If the home serves four or more individuals or if the home has three or more stories including the basement and attic, there shall be at least one smoke detector on each floor interconnected and audible throughout the home or an automatic fire alarm system that is audible throughout the home. The requirement for homes with three or more stories does not apply to homes licensed in accordance with this chapter prior to November 8, 1991. The smoke detectors located second floor were interconnected, however during the time of the inspection, the manager testing the detectors did not hold the alarm button long enough for the interconnection to go off. This issue was not discovered until Thursday April 14th, 2022, when maintenance went to the location to fix the issue. The smoke detectors are interconnected. o LSS Managers will be educated to hold the smoke detectors alarm button down for 5 to 10 seconds when testing the alarms that are to be interconnected. o LSS Compliance Coordinator will verify & track alarms are interconnected at each location that have 3 or more stories. o Identify on house floor plans where smoke detectors are interconnected. 04/29/2022 Implemented

From July 2021 to November 2021, there were three incidents in which the medication was not administered as prescribed. Individual #1 was to receive milk of magnesia if there was no bowel movement in three days. There were two incidents in July 2021 in which Individual #1 went three days with no bowel movement; but did not receive any milk of magnesia. In November 2021, Individual #1 received milk of magnesia on day four of no bowel movements, a day late.A prescription medication shall be administered as prescribed.Moving forward when a consumer goes to PCP, ER and/or an urgent care center due to BM's the prescribed medication will continue to be administered unless a discharge order from a physician is obtained. 04/29/2022 Implemented

Due to the nature of how this problem was discovered LSS is not able to develop a plan of correction to resolve this issue. LSS feels the way this was discovered was a rights violation due to the consumer not being present to give consent for the inspector to search his personal dresser drawers as the drawers were closed.
