| LSS developed an Assessment Summary Notification Letter for team members which indicates date that the assessment was sent to Plan Team Members (see attachment 7).
LSS Program Case Coordinators are responsible for sending team member the Assessment Summary Notification Letter 30 days prior to the ISP meeting date. A copy of the letter will be filed in the consumer's program book.
