The directions for Clindamycin Phosp 1% lotion on the Individual #1's medication log reads apply to affected areas of trunk twice daily at 8A and 8P.The original container for prescription medications shall be labeled with a pharmaceutical label that includes the individual's name, the name of the medication, the date the prescription was issued, the prescribed dose and the name of the prescribing physician. The med log was changed to read exactly the same as the med label. Both read to apply to the face and trunk twice daily at 8A and 8P. [CEO or designee will immediately review all medications logs and all pharmaceutical labels for all individuals in all the community homes to ensure all individuals are receiving the prescribed dosage of all medications and that directions are correct on the medication logs. The CEO or designee will continue to review doctors orders, prescription labels and med logs for all individuals at least monthly for the next 6 months to ensure there are not any discrepancies. Monthly reviews will continue until there are 6 consecutive months of error free documentation. Documentation of all reviews will be maintained and reviewed by the CEO. 6400.163(c)The Psychiatric reviews for Individual # 1, who is prescribed psychotropic medication, were completed on 3/3/15 and 8/31/15. If a medication is prescribed to treat symptoms of a diagnosed psychiatric illness, there shall be a review with documentation by a licensed physician at least every 3 months that includes the reason for prescribing the medication, the need to continue the medication and the necessary dosage.Because the Doctor's office was responsible for cancelling the appointment, we thought documentation verifying that was sufficient. We now know that it is not. We have instructed all supervisors that in the event a Doctor's office (psychiatrist) cancels a required appointment, we will ask the doctor if he will complete the med review form. If that is not possible, we will go to the family doctor for the med review. This was addressed at the management meeting held on 12/16/2015. [Immediately, CEO or designee will review the most recent medication review documentation for all individuals and ensure appointments are scheduled to ensure reviews will be completed timely. Supervisors will schedule all medication review appointments to ensure that reviews are completed within the required timeframes. Management staff will be informed of the scheduled appointments by the supervisors and whether or not the individuals attended the scheduled appointments. Missed appointments will be made up within the required timeframes to ensure Individuals are receiving the required medication reviews. CEO will review a 25% sample of medication review appointment documentation at least quarterly for 6 months to ensure reviews are completed timely.
