6400.181(e)(4) | Individual #1's assessment, completed 11/1/19 does not include the individual's need for supervision. | The assessment must include the following information: The individual's need for supervision.
| The Need For Supervision Assessment was revised. It now includes an area where the Program Specialist will describe why 1:1 supervision is required, if that is the case.
The Need For Supervision Assessment was also revised to match the Health and Safety Focus Tool, in chronological sections and topics, to allow the Program Specialist to utilize the information gained through the assessment, to summarize each section of the Health and Safety Focus Tool.
In the Health and Safety Focus Tool the Program Specialist will recommend and describe for each specific activity/section the supervision or physical assistance the person requires. The Assessment and the Focus Tool will be forwarded to each team member prior to the ISP for review and contemplation of the Need for Supervision / assistance the Program Specialist recommends in each area.
At the ISP meeting, the recommendations will be discussed and finalized. The Health and Safety Focus Tool will then become the Need for Supervision Statement for that person¿s ISP year, or until a needed revision is made.
