Floors, walls, ceilings and other surfaces shall be free of hazards.The dried leaves were removed on 12/16/16 no longer creating a potential for a fall hazard. (See picture attachment) Woods contracts with landscaping company who are responsible for leaf removal. 6400.101The exit doors were equiped with a 15 second delayed system, which was inoperable and appeared to be faulty causing the door not to open when pushed. Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. Woods maintenance department disabled the mag locks on the delayed system disabling the delayed system for use. (See completed work order dated 12/16/16) 12/06/2016 Implemented
