6400.68(b)On 2/8/18 at 2:31 PM, the hot water temperature measured 124.3 degrees Fahrenheit in the bathtub in the bathroom on the left side of the hallway in the home. Hot water temperatures in bathtubs and showers may not exceed 120°F. Hot water temperature was turned down on February 8, 2018 by the maintenance department. Staff will check the water temperature on a daily and record it on a hot water sheet. [Within 30 days of receipt of the plan of correction, the Director of Residential or designee shall educate all staff persons responsible for checking water temperature on the agencies procedures for checking, reporting, adjusting and documenting hot water temperatures to ensure an accurate reading and documentation of hot water temperature measurement and the hot water temperatures in bathtubs and showers does not exceed 120°F. 6400.101On 1/6/16, the front door and back door leading to the outside of the home were equipped with a coded key pad locking mechanism preventing immediate egress from both exits of the home. In addition, the front porch was entirely enclosed with chain link fencing. The gate leading from the front porch was equipped with a combination lock preventing immediate egress from the front porch of the home.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. The fence in front of the house is no longer present. It was removed on 6/27/16. UCIP decided to discharge that individual on 6/7/16. [Individual #1 was discharged as of 6/8/2016.
