Prospectus Berco Human Resources department explained that reports for Physical Examination due dates excluded staff person. Employee clock number, based on hire date, was two digits. 2380.53(b)Poisons shall be stored in their original labeled containers. In a supply closet located in the hallway between program components (rooms) 3 and 4, there was an unlabeled spray bottle containing a blue liquid resembling window cleaner.Poisonous materials shall be stored in their original, labeled containers.A spray bottle containing a blue liquid was found in locked storage closet at end of the hallway. The spray bottle was part of Whistle Maintenance janitor's cleaning supply. The spray bottle including the blue contents (window cleaner) was immediately discarded on September 1st. The janitor last day working in building was August 30th. The janitor has since resigned. Agency no longer has a cleaning contract with Whistle Maintenance. Prospectus Berco has signed a contract with City Wide Maintenance to clean Kutztown Road building. Debi Dawkins Purchasing and Facilities Director met with the owner and reviewed the importance of the Regulation and why it is in place. He was given a copy of the regulation. Ms. Dawkins posted the Regulation with a note detailing storage of poisonous materials, including cleaning supplies. Checked all cleaning supplies in building and there are no poisonous materials in an unlabeled container in the building. 09/09/2021 Implemented
2380.126(a)(11)The Medication Administration Record (MAR) for Individual #1 does not contain a diagnosis or purpose for the medication Docusate Sodium 100mg, take one cap three times per day at 7am, 12pm and 7pm.A medication record shall be kept, including the following for each individual for whom a prescription medication is administered: Diagnosis or purpose for the medication, including pro re nata.Chapter 2380 126(a)(11) MAR did not have a diagnosis or purpose for the Docusate Sodium listed with the medication. Person 1 is prescribed one medication that is administered at the Day Program. The diagnosis was listed with other diagnosis at the bottom of the MAR. This regulation is important in event he is prescribed more than one medication and or has multiple diagnosis. On morning of September 2nd Director went on the AWARDS system MedSupport for Person 1 and entered the purpose and diagnosis with the Docusate Sodium. This purpose/diagnosis appears with each medication on the MAR form. This now part of the form and is included in the electronic record keeping.
