| 6400.144 | Individual #1's current and prior physical, ISP and assessment all provide special diet instructions which include a heart healthy diet and mechanical soft-chopped. A review of the Individual's financial records shows that they are purchasing foods at restaurants such as Burger King, McDonalds, Domino's, and Chinese Buffets as well as snack items such as Pringles, Tasteycakes, Fudge Rounds, Honey Buns, Zebra Cakes, etc. The Individual has not been trained or educated on the special diet instructions regarding a heart healthy diet.
Individual #1's ISP and Dental Hygiene Plan state that they are to use alcohol-free mouthwash. Upon home inspection, the individual was found to be using Equate Antiseptic mouth rinse which contains 21.6% alcohol. The mouth rinse was also expired as of 7/2022. | Health services, such as medical, nursing, pharmaceutical, dental, dietary and psychological services that are planned or prescribed for the individual shall be arranged for or provided.
| Nurse provided training to GW on 3.13.23 to educate him on his heart healthy diet. Please see attachment B. Medical Compliance Specialists checked other plans for individuals on caseloads to ensure education was provided for specialized diets. No issues were found.
Manager El purchased Alcohol Free mouthwash for on 3.10.23 with an expiration date of 10/25, see attachment C. Site Supervisors checked all OTCs on 3.13.23 to ensure OTC matched orders and were in date. No issues were found. |
