| 6400.144 | Individual #1 had a dental apt in 9/17/2020 and a follow up scheduled in April 2021. The dental apt in April was missed and the agency called July 14, 2021 to reschedule that apt. At that time an apt of Aug 4, 2021 was provided. Individual had to miss that dental apt b/c of a supports coordinator meeting and now is scheduled for Aug 12, 2021. | Health services, such as medical, nursing, pharmaceutical, dental, dietary and psychological services that are planned or prescribed for the individual shall be arranged for or provided.
