6400.72(b)There was a tear in the screen on the sliding glass doors leading from the den to the outside. Screens, windows and doors shall be in good repair. 1. The provider will ensure that all screens/all physical aspects of the home are in good repair by completing the residential facility inspection on a biannual basis in June and December as well as routine checks of the site by all staff. Needed repairs are to be reported through the internal online maintenance reporting system, Manager Plus. 2. Immediate notification of deficiency was made to Karen Roberts, Maintenance. Maintenance on site on 10/22/18 to determine that a complete replacement was needed. Full repair was completed on 10/24/2018 with replacement of the screen. 3. To prevent reoccurrence of this issue the Operations Director shall utilize the state issued self-inspection form as outlined in regulations noting any deficiencies as well as the bi-annual internal facility inspection report. Deficiencies noted will be recorded and submitted for installation or repair to the maintenance department. Any issues noted to be a potential hazard shall be reported and rectified immediately. 4. Operations Directors/Program Specialists shall complete the stated forms, with monitoring and approval provided by the Regional Director, the forms shall be completed as outlined reviewed by the Operations Director/Regional Director. Any instances of noncompliance shall be logged into the Manager Plus system used by agency maintenance personnel or in instances of a potential hazard notification shall be made via telephone with resolution expected immediately. 10/22/2018 Implemented
6400.106Late furnace inspection/cleaning - The furnace was inspected and cleaned on 8/01/16 and not again until 10/24/17.Furnaces shall be inspected and cleaned at least annually by a professional furnace cleaning company. Written documentation of the inspection and cleaning shall be kept. 1. The provider will ensure that furnace inspections are completed in accordance with regulation. 2. No immediate action was required as the cleaning had occurred but was late. 3. To prevent reoccurrence of this issue, The Office Manager will ensure that inspections are completed on an annual basis by establishing a schedule with the designated service provider. All homes will be scheduled for cleaning during the same month. Additionally all appointments will be entered into the Outlook calendar system so that a reminder is generated two weeks prior to the scheduled appointment. 4.
