6400.163(c) | The 12/14/18 medication review for Individual #1 did not have if the current medications prescribed should or should not be continued. | If a medication is prescribed to treat symptoms of a diagnosed psychiatric illness, there shall be a review with documentation by a licensed physician at least every 3 months that includes the reason for prescribing the medication, the need to continue the medication and the necessary dosage. | Based on input from licensing, we have revised our medication review form to be more in line with provider community. The form requires notation of each psychiatrist prescription separately rather than attaching a MAR alone. This insures the need to continue is clearly associated with prescribed medications. We have also made relevant sections more prominent on the form for oversight. The updated form is attached as number 6; we do not have an example used since the update of the form and distribution to programs on 4/19/2019. A review completed since licensing will be sent as an addendum. |
