6400.62(a) | On 2/14/2024 at 11:50am, a 19-ounce can of Lysol disinfectant spray was observed under the kitchen sink. The label on the can stated to contact a Poison Control Center of a doctor if sprayed in eyes or on skin. Individual #1's individual support plan that was last updated on 12/7/2023 states that the individual requires constant supervision while handling poisons and that poisons must remain locked for the individual's safety. | Poisonous materials shall be kept locked or made inaccessible to individuals. | An audit was conducted on 2/23/2024 by the Executive Director to ensure that poisonous materials were locked up in the staff office, inaccessible to the individual (Attachment A). The Executive Director reviewed the monthly site checklist to ensure regulation 6400.62(a) is being monitored at monthly site checks. This was listed on the site checklist (Attachment B). Current staff were educated by their immediate supervisor via a phone meeting on 2/26/2024 to educate them on the ISP and 6400.62(a) (Attachment C). |
