6400.104 | Individual #1's letter to the fire department dated 8/31/2018 stated he requires verbal and physical assistance to exit the home during a fire. It also states an Alert that he is obese. Individual #1's assessment updated 6/5/2019 and current Individual Support Plan (ISP) 6/12/2019 both state he is independent during fire drills and in the event of a fire to exit the home. | The home shall notify the local fire department in writing of the address of the home and the exact location of the bedrooms of individuals who need assistance evacuating in the event of an actual fire. The notification shall be kept current.
| On 7/30/19, Administrative Coordinator revised Individual #1 's notification letter and mailed it. See attachment (Sunset Ave #1). On 8/1/19 and 8/7/19, Assistant Vice President retrained Administrative Coordinator, Residential Coordinator, and Program Specialists, on Regulation 6400.104 and Notification to Local Fire Department section of the Regulatory Compliance Guide. It will be the responsibility of the Program Specialist to inform Administrative Coordinator any changes in individual¿s ability to evacuate in the event of a fire. See attachment (Sunset Ave #2). Program Specialists reviewed all individual¿s assessments, ISP¿s, evacuation plans, and notification letter to local fire department for discrepancies. See attachment (Sunset Ave #3). |
