The cleaners were accessible prior to entry to the other unit. Based on the licensing review of November 8-9, 2021, the CLA Director, on 11/10/21, retrained the Site Supervisors responsible for the sites, as well as the Assistant Directors about the importance of keeping poisonous materials properly secured. The ISP, including annual updates and revisions under § 6400.186 (relating to ISP review and revision), must include the following: A protocol to address the social, emotional and environmental needs of the individual, if medication has been prescribed to treat symptoms of a diagnosed psychiatric illness. Based on the licensing review of November 29-30, 2016, on 12/1/16, the Program Supervisor responsible for the site as well as the Assistant CLA Directors, were sent a memo from the Director on the importance of assuring that a SEEP is completed for all individuals on a psychotropic medication. A SEEP was completed for this individual on 12/8/16. It is the responsibility of the Program Supervisor/Specialist to assure that each person on a psychotropic medication has a SEEP, social/ emotional, environmental plan in place and this will be reviewed by the Assistant CLA Director to assure compliance. 12/08/2016 Implemented
