The ISP, including annual updates and revisions under § 6400.186 (relating to ISP review and revision), must include the following: A protocol to address the social, emotional and environmental needs of the individual, if medication has been prescribed to treat symptoms of a diagnosed psychiatric illness. The individual's charts residing on the second floor of 7965 were not reviewed by licensors. Since the individual residing on the first floor of 7965 Algon is on a psychotropic medication and their chart was audited by licensors, we believe that this citation more accurately reflects the first floor, not the second floor of Algon. Based on the licensing review of November 29-30, 2016, on 12/1/16, the Program Supervisor responsible for the site as well as the Assistant CLA Directors, were sent a memo from the Director on the importance of assuring that a SEEP is completed for all individuals on a psychotropic medication. A SEEP was completed for this individual on 12/8/16. It is the responsibility of the Program Supervisor/Specialist to assure that each person on a psychotropic medication has a SEEP, social/ emotional, environmental plan in place and this will be reviewed by the Assistant CLA Director to assure compliance. 12/08/2016 Implemented

6400.66The lightbulb over the back entry door was out.Rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps and fire escapes shall be lighted to assure safety and to avoid accidents. This citation was resolved on 9/16/15 and the light bulb in the back entry was replaced. A newly revised CLA Site Checklist is completed on a weekly basis by the site supervisor to assure that all surfaces are well lit. On 9/17/15 all of the PATH Program Supervisors/Specialists responsible for the site as well as the Assistant CLA Director were retrained on the importance of keeping the site well lit. It is the responsibility of the Program Supervisor/Specialist to assure that each household is lit appropriately to assure safety. 09/16/2015 Implemented
6400.68(b)The water temperature in the main hallway bathroom was 124 degrees. Hot water temperatures in bathtubs and showers may not exceed 120°F. This citation was resolved on 9/18/15 when The CLA Director, with support from the facilities department had conventional valves at each site replaced with thermostatic mixing valves to assure the water temperature will not exceed 120°. The thermostatic mixing valves come complete with built in temperature gauge and check valve built in. A newly revised CLA Site Checklist is completed on a weekly basis by the site supervisor to assure that the water temperature does not exceed 120° In the event of a temperature higher than 120° the site supervisor will assure that consumers and staff do not use the site hot water until the valve is fixed or replaced. 09/18/2015 Implemented

The Nurse Manager will facilitate bi-weekly meetings with each healthcare assistant and Nurse team for the primary purpose of reviewing medical appointments for each individual per caseload. For all medical follow up needed, the healthcare assistant who facilitates the appointment will inform the Nurse or Nurse Manager within (1) business day and a follow up appt. will be immediately scheduled. The protocol must include the current level of independence and the method of evaluation used to determine progress toward the expected outcome to achieve the higher level of independence. The individual's assessment was updated on 9/27/13 and further revised on 11/6/13 to address unsupervised time. To assure compliance in this area, the CLA Assessment document was revised and implemented in October 2013 with revisions to the Independence section (IV.).
