If they are not a resident of the Commonwealth, a Federal Bureau of Investigation (FBI) Criminal History Record check will be completed within five working days after their date of hire. The HR Operations Manager will be responsible to review all applications to assure that all individuals in the future have an FBI check completed when they are not a resident of Pennsylvania consistent with 6400 regulations. This citation was resolved on 3/26/19 when an applicant's residency was properly indicated and reviewed by the HR Operations Manager consistent with 6400 regulations. 03/26/2019 Implemented
6400.76(a)The kitchen chairs had 2 chairs that have damage on the soft cushions. Furniture and equipment shall be nonhazardous, clean and sturdy. Based on the licensing review of February 26-27, 2019, the CLA Director retrained the Residential Site Supervisors responsible for the sites as well as the Assistant Directors on 3/6/19 about the importance maintaining clean and sanitary conditions at all sites. It is the responsibility of the Residential Site Supervisor to assure that each household is in good repair on a weekly basis. A CLA Site Checklist is completed on a weekly basis by the Residential Site Supervisor to assure that all surfaces are clean and in good repair. When items are not clean, worn or broken, the Residential Site Supervisor will take corrective action to assure compliance with this regulation. To further assure regulatory compliance, Assistant Directors will conduct unannounced quarterly site visits that will be monitored by the CLA Director.This citation was resolved on 3/19/19 when two new chairs were purchased for this site. 03/19/2019 Implemented
